Important Things to Know for 2011-2012

  1. The Floydada ISD Cafeteria plan year is September 1, 2011 through August 31, 2012.


  2. Flexible Spending Accounts: MGM is the Third Party Administrator (TPA) for Floydada ISD. Medical Reimbursement participants pay a fee of $1.50/month for flex card. Direct deposit is available for flexible spending accounts.


  3. Disability Plan: Unum Educator Salary Protection plan - all new coverage or changes in coverage are subject to pre-existing condition limitations. Coverage is guarantee issued. See Benefit & Forms/Disability for more information.


  4. Voluntary Group Term Life: Unum will remain the provider for voluntary group term life for Floydada ISD. For rates and plan summary see Benefits & Forms/Life/Voluntary Life.


  5. Permanent Life: Texas Life is offering a permanent term life. This may be purchased for employees, spouses, and or children. For more information see Benefits & Forms/Life/Permanent Life/Highlights Sheet.


  6. Vision Plan: Block Vision will remain the vision provider for Floydada ISD. See Benefits & Forms/Vision/Plan Information for details.


  7. Dental Plan: Guardian will remain the dental provider for Floydada ISD. See Benefits & Forms/Dental/Plan Information for details on this benefit.


  8. Accident Plan: American Public Life is Floydada ISD’s provider for on and off the job accident coverage. This coverage is guarantee issued to employees, spouses and children.


  9. Cancer Plan: Philadelphia American is the cancer provider for Floydada ISD. This coverage is competitively priced and requires an application for new coverage.


  10. Heart & Stroke Plan: Allstate/American Heritage provides Floydada ISD employees a heart disease, heart attack, stroke policy. Application is required for new coverage.


  11. Product offerings to Floydada ISD employees include: Flexible Spending Accounts with MGM Visa Card, Unum Disability, Unum Life, Texas Life, Block Vision, Guardian Dental, American Public Life accident, Philadelphia American/CSO Cancer, and Allstate/American Heritage Heart Stroke plan.
