IMPORTANT: Effective beginning on January 1, 2011, over-the-counter medicines or drugs (e.g. Advil, Ibuprofen, cough syrup) are not eligible for reimbursement under an FSA, without a doctor’s prescription. Insulin is the only medicine that doesn’t require a prescription. Also supplies that you need for medical care (e.g. contact lens solutions, bandages for wounds, thermometers) will continue to be eligible for reimbursement.
As such, upcoming enrollment in your employer’s flexible spending account should take into consideration as of 1/1/2011, over the counter items will no longer be reimbursable under the health FSA.

Important Things to Know for 2010-2011

  1. The Floydada ISD Cafeteria plan year is September 1, 2010 through August 31, 2011.


  2. Flexible Spending Accounts: MGM is the Third Party Administrator (TPA) for Floydada ISD. There is a new MGM Flex Visa Card for all Medical Reimbursement participants for a fee of $1.50/month. Direct deposit is available for flexible spending accounts.


  3. Disability: Unum Educator Salary Protection plan - all new coverage or changes in coverage are subject to pre-existing condition limitations. Coverage is guarantee issued.


  4. Term Life: Unum is the provider for voluntary group term life.


  5. Individual Life: Texas Life is offering a permanent term life. This may be purchased for employees, spouses, and or children. See Benefits & Forms, Life, Permanent Life, Highlights/Texas Life Brochure for more information.


  6. Vision: Block Vision is the new vision provider for Floydada ISD beginning September 1, 2010. See Benefits & Forms, Vision, Plan Information for details on this new enhanced benefit.


  7. 7. Dental: Guardian is the new dental provider for Floydada ISD beginning September 1, 2010. See Benefits & Forms, Dental, Plan Information for details on this benefit.


  8. Accident: American Public Life is Floydada ISD’s provider for on and off the job accident coverage. This coverage is guarantee issued to employees, spouses and children.


  9. Cancer: Philadelphia American is the cancer provider for Floydada ISD. This coverage is competitively prices and requires an application for new coverage.


  10. Heart: Allstate/American Heritage provides Floydada ISD employees a heart disease, heart attack, stroke policy. Application is required for new coverage.


  11. Product offerings to Floydada ISD employees include: Flexible Spending Accounts with MGM Visa Card, Unum Disability, Unum Life, Texas Life, Block Vision, Guardian Dental, American Public Life accident, Philadelphia American/CSO Cancer, and Allstate/American Heritage Heart Stroke plan.
