Important Things to Know for 2012-2013
Plan Year Septemeber 1, 2012 to August 31, 2013
- NEW National Benefit Services will be the new Section 125 Third Party Administrator (TPA) for Floydada Independent School District. A new NBS Flex Card will be issued to those employees who participate in the Flex Card option. Direct deposit is available. Please remember to keep your receipts if you use your Flex Card. The medical reimbursement annual maximum is $2,500 / plan year. The dependent care reimbursement annual maximum is $5,000 / plan year, if married, filing jointly and $2,500 / plan year if filing single.
- If you currently participate in the medical or dependent care reimbursement flexible spending accounts, you MUST log in and elect this benefit every year. If you do not re-enroll annually, your participation will automatically be waived.
- Unum Voluntary Group Term Life will reamin the Voluntary Term Life / AD&D carrier for Floydada Independent School District. Guarantee Issue is only available for new employees of the District, as long as coverage is elected within the 31 day new hire enrollment period.
- Benefits and rates will remain the same for the following coverage types: OraQuest Dental, Block Vision, Unum Educator Disability, Philadelphia American Cancer, American Public Life Accident, Allstate Heart & Stroke, Unum Voluntary Group Term Life / AD&D and Texas Life Individual Life.
